Data Collection Form: Romantic Atlas

Commissioned by the Department of the Romantic Atlas and the Philadelphia Special Romantic District, this data collection form is a mashup of familiar voter registration and IRS forms loosely following Federal paperwork guidelines. The Romantic Atlas is the brainchild of Michelle Schmitt and aims to use data collected through such forms to map the density of Philadelphia area courtship habits.
